Rehab to Home Program The Pavilion has come to be recognized as one of the leading providers of advanced nursing and rehabilitation services in the area. We offer a broad range of intensive therapeutic services designed to maximize the functional abilities of our residents, enabling them to reach their highest level of independence as quickly as possible. The majority of our residents return home to lead productive lives. As much as possible, family members are encouraged to participate in the rehabilitation process to help prepare for the resident's return home.
Physical Therapy Physical therapy is provided by a Licensed Physical Therapist and licensed staff who utilize modern techniques and equipment to provide rehabilitative services. Many residents who are discharged from a hospital and are not yet ready to return home may receive physical therapy services in The Pavilion skilled nursing facility. Physical therapy includes gait training with or without an assistive device; transfer training from sit to stand, from chair to bed, from chair to commode, and in and out of a car; standing balance retraining; range of motion training; strength training; coordination training; reduction of pain; healing of wounds; and reduction of joint contractures. A physical therapist can also participate in performing a home assessment to make recommendations for resident safety and independence in achieving a home goal.
Occupational Therapy Occupational therapy is also provided by licensed staff who seek to improve... Occupational therapists seek to improve a resident's ability to perform activities of daily living, prevent and restore loss of function, and facilitate successful adaptation to the aging process. Activities of daily living include tasks that a person performs daily such as, feeding, bathing, dressing, grooming, toileting, transfers, and home management activities.
Speech Therapy Speech therapy addresses more than just speech. It assists individuals in regaining the ability to produce and understand speech and language, as well as cognitive-linguistic and swallowing functions. Some medical conditions that require speech therapy intervention include: stroke, head injury, Alzheimer's, head and neck cancer, Multiple Sclerosis, Parkinson's and other neurological disorders.
Respite Care
Short term stays are available for a weekend, week, month or season. It's the perfect resource for individuals who don't want to leave mom and dad at home alone. Many caregivers and their loved ones who have utilized the program perceive it as a vacation for both parties. Social activities, food, housekeeping and laundry services are all included. And, most importantly, caregivers need not worry while they travel on business or vacation. Their loved one is in a safe, secure environment where they can stay for a few days, a few weeks or more.
Hospice The Pavilion contracts with Hospice agencies to provide unique, comfort-oriented care for terminally ill residents who are no longer seeking curative treatment. Hospice incorporates an interdisciplinary team of physicians, nurses, aides, chaplains, social workers, therapists (for physical, speech and occupational therapy services), and volunteers who provide individualized health care and comfort to the resident and family. Hospice care services are available 24-hours a day, seven days a week. Volunteers are an essential part of hospice care. Typical services provided include medications, medical supplies and equipment services, therapies as part of care plan and counseling and support.
