Rehab To Home


The Pavilion Rehab and Skilled Care Center offers a specialized Rehab to Home program. It is our goal to help a person return to their own home as quickly and safely as possible. We offer Physical Therapy, Occupational Therapy and Speech Therapy in our newly renovated therapy room. We focus on individual and group therapy sessions. Our dedicated team of therapists, nurses and physicians work together towards the patient’s goal of returning to their own home.

 

Our rehabilitation specialists can help residents regain mobility and increase their range of motion. We can improve their muscle strength as well as help them relearn basic daily living skills such as dressing, bathing, eating and swallowing. As much as possible, family members are encouraged to participate in the rehabilitation process to help prepare for the resident's return home.


The Pavilion offers newly renovated Private Rehab Suites for your short term stay. Once a patient is released from the hospital they will be escorted into a beautifully decorated private suite with a private bathroom. Our Private Suites offer flat screen TVs mounted on the wall, free local telephone services, new carpeting, window treatments and recliners. Our Private Rehab Suites are located on our Rehab unit which is located at the entrance of our building. With all our Rehab to Home patients staying in this unit it becomes a community of friends all working towards the same goal of returning to their own homes.
