How can I be admitted to The Pavilion? If you or a loved one is coming from the hospital you will need to let the hospital social worker know that you want to be admitted to The Pavilion. They will make sure all the paperwork is completed and notify the facility staff about the admission. If you or a loved one is coming from a home like setting, you will need to contact your primary physician to get orders for admission. The doctor must also complete and sign a history and physical within 5 days of your admission. The Pavilion social worker will complete the PASS-ID and LOC.
How will I pay for care in a skilled nursing facility? We accept Medicare, Medicaid, Private Pay and Private Insurance. We also accept Private Pay long term care insurance when applicable.
Does Medicare pay for skilled nursing care? Yes, Medicare pays for skilled nursing care if you had a qualifying three day stay in the hospital prior to admission to The Pavilion. You must be receiving a skilled service such as physical or occupational therapy or skilled nursing care such as wound care or IV therapy. How much & how long does Medicare pay? If eligible, Medicare Pays 100% for days 1-20. For Days 21-100 the patient is responsible for the co-payment of 137.50 per day. Please note very few people receive 100 days of skilled nursing care. It is our goal to have a person rehabilitated and back in their own home or to their prior level of function as quickly as possible. Are families and pets allowed to visit? Are there set visitation hours? There are no set visitation hours and families are always welcome. Pets are also welcome and encouraged to visit. Pets must be on a leash and shot records must be turned in at the front office. Can I go to family reunions, out to eat or visits? Yes, we encourage you or your loved one to stay involved with your families and your community.
